Sports Physicals FAQs:
Do athletes need to get a new physical for each sports season? NO! If your student played a Fall 2023 and or Winter 2023-24 PHSD sport, they do not need to get another doctor's physical. This physical (dated after 6/1/23) is valid for the entire 2023-24 school year. Please login to Planet HS to complete the spring parent/guardian re-certification form.
Is there a cost for sports physicals? No! PHSD offers FREE sports physicals on select dates before each sports season.
Who can get a sports physical? Only registered PHSD students in grades and charter school students in grades 7-12 who are in the student registration database (PH residency has been verified) are permitted to attend sports physicals.
Where are the physicals held? At the Penn Hills High School and Linton Middle School Nurse's Offices. You will enter through the Main Entrance at both schools.
Who is the doctor? All sports physicals are conducted by our school physician, Dr. Robert Crossey.
How long will the doctor be at the school on each physical date? The doctor will stay at least an hour but only as long as needed to see everyone who reports for physicals on that date. Please arrive by the end time listed on the physical date form to ensure you are seen by the doctor.
Can my student get a physical from their own doctor? Yes. Students and families may choose to get a physical from their own pediatrician/family doctor. The date on the physical must be after June 1, 2023 to be valid for the 2023-24 school year. We cannot accept physicals before this date. One physical is good for the whole school year. No need to get another physical before each season. Here is the form to have the doctor complete. You will upload a photo of this signed form under Section 6 in Planet HS/Student Central.
Are sports physicals required to attend summer/offseason workouts? No, students are welcome to attend open gyms, workouts, and optional practices in the offseason before having a sports physical. Please share any health concerns with the coach(es) so they are aware.
What is the deadline to get a sports physical? In order to start official practices or tryouts for your team, you must have a doctor's physical AND all forms must be complete on Planet HS/Student Central.
STEPS FOR COMPLETING ONLINE PHYSICAL FORMS:
- Go to www.planeths.com. If you already have an account, please login to your existing account. If you don't remember your login, click forgot password.
- If you do NOT already have an account, click "Create New Account." Choose Parent or Student based on who is creating the account. Students and parents MUST have separate accounts with different/unique email addresses.
- Click on “Linked Accounts” and link to your parent’s or student’s account.
- Click on “Athletic Forms” and start electronically completing/signing the forms.
- Upload a photo of your signed doctor’s physical exam under Section 6 (must be dated after 6/1/23)
