FREE Spring Sports Physical Dates 2025-26

All forms must be signed by the student and parent/guardian in Student Central before getting a physical from the school physician. We will have Section 6 paper forms available at the school for the doctor to sign (you do not need to bring one with you).
If an athlete played a Fall or Winter sport, they do NOT need to get another physical exam. They just need to do the re-certification form in Student Central.
